Diabetes Management

If you are living with diabetes, there is a lot you need to know. With the proper education and support, you can take control of your diabetes instead of your diabetes taking control of you. Through appropriate treatment options, basic information and taking an active role in the management of your diabetes, you can a lead full, healthy and productive life.

If left unmanaged, Diabetes can often lead to numerous debilitating and even life-threatening complications. Certified by the American Diabetes Association (ADA), The Diabetes Center at Holy Name Medical Center provides the highest standard of education for patients about behavior change, prevention and care during complications. Our dedication is to help people living with diabetes self manage their disease in hopes of helping to prevent and/or delay possible complications. Patients with diabetes receive counseling from a registered nurse/certified diabetes educator on insulin self-administration, blood sugar monitoring, exercise, health, maintenance, and the importance of routine physical follow-up. With the combination of education and emotional support, we strive to promote the long-term benefits for chronic disease management to better help our patients avoid long-term complications from diabetes.

The Diabetes Center Provides:
  • Comprehensive education to people with metabolic syndrome, pre-diabetes, Type 1 or Type 2 Diabetes, or gestational diabetes.
  • Instructions on how to use a blood glucose meter, along with the why, and when to test, as well as keeping a blood sugar log.
  • Education on meal planning, carbohydrate counting, food diary, diet and exercise.
  • Follow up care to assess blood sugar management (A1c), and collaboration with physicians on medication adjustments.
  • Continued support to motivate/ empower patient with tools of self-care.
