Telemedicine Visits

Dear Valued Patient,

At Holy Name Medical Partners, we want you to know that you and your family's well-being is a top priority for us. Rest assured, we meet or exceed CDC-mandated procedures for health and safety, and we are taking every precaution to ensure you are getting the same high quality, compassionate, personalized care that you expect from us.

To that end, we have expanded our practice's capabilities to offer telemedicine visits to those who are in need of routine care to include:

  • blood pressure checks.
  • medication refills.
  • most routine follow-up care.

Simply call your physician's office as you would normally do to schedule an appointment. You will be advised if a telemedicine visit is an option for your specific healthcare concern. If you are told that you should come to the office for a regular consultation, take comfort in knowing our practice environments are safe and that all CDC regulatory guidelines are being followed.

These same telemedicine capabilities are also in place to proactively screen and identify symptomatic patients of COVID-19. We ask that you call your physician's office first, and arrangements will be made for a telemedicine visit, where you will be instructed on the next steps for proper testing, monitoring and follow-up care.

Outlined below is further guidance to help you protect yourself, your families and our healthcare workers.

Patients 80 Years of Age and older

It is safest for you to limit your time outside your home environment whenever possible. We ask that you rely on family and friends to help with shopping. Please ensure you have a sufficient supply of all medications and medical supplies. Take advantage of our telemedicine services for your routine care. Simply call your physician's office and you will be advised on how to proceed.

We recommend that you stay active indoors to help support your immune system and mental health.

Patients aged 60 or older with underlying illnesses

If you have underlying illnesses such as COPD, diabetes, lung disease, etc., please remain diligent in practicing social distancing. If you feel ill, please call your doctor first before heading to the practice.

Healthy Patients 60 or older

Our practices are continuing to offer well-patient visits and in-office procedures as previously scheduled. Call the doctor's office to see if your service can be completed with a telemedicine consult. If at any time you have cold or flu-like symptoms, let your doctor know immediately.

We recommend you go outside, take a walk and breathe fresh air. It is important to stay active and support your overall wellbeing.

Patients aged 19-59

Our practices will continue to see patients for well-visits, routine follow-ups and minor sick visits that are not respiratory in nature. When you call for an appointment, you will be advised if telemedicine is an option. If at any time you have cold or flu-like symptoms, let your doctor know immediately.

Pediatric Patients (Newborn to age 18):

Our pediatric and family practice offices are open to see all patients for sick and well visits. Parents or guardians that may have cold or flu-like symptoms are asked to call the office prior to coming in with your child so we can better understand their symptoms.

Staying safe and healthy is very simple:

It is important to understand that practicing social distancing and self-isolation is the best way to fight the spread of Coronavirus. According to the CDC, one person unknowingly infected has the potential to spread it to two other people who will then spread it to two more people, compounding the number and spreading the virus exponentially through their community.

Other ways to stay safe and healthy

  • Practice good hand hygiene
  • Drink plenty of water
  • Go outside and get fresh air
  • Exercise, including housework
  • Take a walk
  • Continue to eat a healthy well-balanced diet.
