Classes for the LPN day program begin in September.
Submission of an application form with a non-refundable application fee is required.
The Test of Essential Academic Skills (TEAS), a standardized pre-nursing entrance examination, is administered to measure general academic ability, including math skills, reading comprehension and test-taking aptitude. Candidates must pre-register and pay the non-refundable exam fee at the time of registration. If special accommodations are needed to take the TEAS, candidates should alert the school office at the time of registration. Holy Name Medical Center School of Nursing allows applicants to sit for the TEAS two times in a calendar year with 30 days between each attempt. The waiting time frame provides applicants additional time to prepare for the exam.
Applicants need to have an official copy of their high school transcript, and where applicable, their college transcript, forwarded to the Holy Name School of Practical Nursing. High school equivalency certificates, with scores, should also be forwarded to the school.
Each applicant is required to have a complete physical examination, including a 2-step Mantoux test, CBC, urinalysis and dates of immunizations for admission to Holy Name's School of Nursing. Health forms and proof of immunizations must be submitted to the school office before the first day of class. Students are encouraged to have health insurance and bring proof of insurance to the school nursing health officer. Holy Name's School of Nursing does not provide insurance.
All students are required to have health insurance, proof of which must be brought to the School of Nursing Health Officer, who will file it with the student's health record. The School does not provide insurance.
All students are required to carry current liability insurance.
Pursuant to the Joint Commission Standard, all students must undergo a criminal background check before acceptance into Holy Name's School of Nursing. A criminal history background check, including fingerprinting, is a prerequisite to licensure.
Holy Name's School of Nursing is a drug-free environment. Every candidate is required to submit to a drug test prior to admission and if requested, additional tests once enrolled.
Applications will be accepted on a rolling basis until admission application deadline. After deadline, applicants will be ranked according to TEAS score (highest to lowest, minimum being 60%).
