Holy Name Medical Center (HNMC) has networked with Jersey Health Connect and Relay Health to provide an online service that enables the sharing and exchange of electronic health records - your health records (tests, lab results, prescriptions dosages, x-rays, and much more) accessible to authorized hospitals, physicians, labs, pharmacies, and to you, our patient.
Jersey Health Connect (JHC) is the state's largest health information exchange (HIE) with over 28 health systems and hospitals, physician group practices, long-term care facilities and home health agencies. Throughout northern and central New Jersey, Jersey Health Connect is leading the way by linking patients and providers statewide. JHC works closely with healthcare provides across the State to develop and implement secure systems and processes for sharing clinical information.
The core benefit of Jersey Health Connect for patients is not just recording information from your electronic health record online, but allowing and facilitating that information to follow you.
Follow the directions for setting up an account with Relay Health. You can do so from this page, by clicking on the Register button in the green box. Select 'Register as a Patient' and follow the directions. Once registered and accepted you will be able to begin viewing your results, and add personal information as well. You will also be able to link other family members to your account if that is appropriate.
Jersey Health Connect uses advanced technology to securely connect clinical data and information systems among its member healthcare organizations and providers. This enables clinicians to appropriately and quickly access a patient's medical records in emergency and walk-in settings. This access can reduce errors and enhance care.
From its conception, JHC has been carefully designed to protect patient privacy. There is no central storage of clinical data. JHC utilizes state-of-the-art electronic systems that allow participating healthcare entities in New Jersey to quickly share vital clinical information only with the patient's permission. It is a highly secure system designed to preserve patient privacy. In addition, other safety factors (including data encryption) are used for increased protection.
The JHC system is built on the secure messaging standards created by the Office of the National Coordinator for Health Information Technology and is compliant with the Health Insurance Portability and Accountability Act (HIPAA) and other federal privacy regulatory requirements.
In addition, JHC uses RelayHealth service, which is a web-based service through which providers and patients can communicate securely. The service uses a high level of security to protect the privacy and confidentiality of healthcare information transmitted through its provider-patient messaging service. Unlike e-mail, which relies on multiple file servers distributed across the Internet, Jersey Health uses a single, centrally managed, secure database for all provider-patient communications. All messages are delivered to a Web browser using 128-bit, secure-socket layer encryption. To prevent others from viewing your private information, you should always log out after using this service.
