Admission Procedure for LPN-to-RN Program Track

  • Attend a Holy Name Medical Center Sister Claire Tynan School of Nursing Information Session
  • Complete on-line application by designated deadline. There is a nonrefundable $75.00 application fee for the Holy Name Medical Center Sister Claire Tynan School of Nursing.
  • Register and take the TEAS Exam. The Test of Essential Academic Skills (TEAS), a standardized nursing entrance examination, is administered to measure general academic ability, including math skills, reading comprehension and science knowledge. Candidates for the LPN-RN Program must complete the ATI TEAS Admission exam with a minimum composite score of 60%. Student must register for the TEAS exam in advance.   • Possess a valid unencumbered LPN license.
  • The following required college courses must be completed prior to entering then formal RN program in January. Please Note: These courses must be competed with a minimum grade of C, Anatomy and Physiology I course must have been completed within 10 years of application date.
    1. Intro to Psychology (3 credits)
    2. Anatomy & Physiology I (4 credits)
  • Official transcripts from ALL SCHOOLS ATTENDED (high school, GED, ALL colleges, vocational/technical schools, school of practical nursing, etc.)
  • Transferred science courses required for the nursing curriculum must have been completed within the last ten years with a minimum grade of C.
  • The applicant's application, transcripts, and entrance exam results are all considered in determining acceptance into the LPN-RN Track.
  • If admitted, the student is given credit for the first clinical nursing course in the RN curriculum and enters the RN Program in January. Students then progress through RN Program curriculum as published including all general education and science courses. Please refer to the RN Program tabs on the website for more information on the RN Program curriculum.
  • All admitted students must attend the LPN-RN Pathway Bridge Course which is offered during the Fall semester prior to entering the formal RN program in January. The Pathway Bridge course is a prerequisite requirement to enter the LPN-RN Program.

LPN-RN Program Track Admission Application Key Steps

  1. Completion of an on-line application form with a $75 non-refundable application fee is required.
  2. Contact ALL schools attended including High school/GED and have the schools mail or electronically email the official transcripts to the Holy Name School of Nursing.   3. Mandatory attendance at a school Information Session. To pre-register for an upcoming session, please refer to the Information Session tab on our website.
  4. Register to take the TEAS exam (www.atitesting.com). Make sure the TEAS includes all four components: English, Reading, Science and Math. Once completed contact ATI and have them email us the transcript results. Obtain a ATI TEAS composite score of 60% or higher.
  5. To review your admission checklist login to: http://hnmcsonis.jenzabarcloud.com. Input user name and pin# created when you applied to the Holy Name School of Nursing.
  6. Application deadline for the LPN-RN Program track admission consideration is April 17, 2025. All admission steps must be completed and submitted to the Holy Name School of Nursing Admissions Office by this deadline to be considered for admission.
